Liberty Hospice: Guiding you through your journey with dignity.

Liberty Hospice offers compassionate care to patients facing a life-limiting illness. At its core, hospice care is about you and your family. That’s why we respect the journey and your wishes. In clinical terms, we allow you to make the most of every moment while receiving comprehensive support from trusted professionals. However, in human terms, you or a loved one will be surrounded by kind, sensitive individuals who will be there to help in any and all ways—at all times.

Liberty Hospice serves people 18 years and older, with a life expectancy of six months or less if the disease runs its normal course. Hospice eligibility needs to be certified by a physician (most often the patient’s own doctor) and the Hospice Medical Director.

Our Medical Director works directly with your Hospice Care team, which also includes Social Workers, Chaplains, Skilled Nursing Professionals, Hospice Aides and Volunteers. Liberty Lutheran’s caring staff is available to support the patient and his or her family in any way possible

    Liberty Hospice understands that every circumstance is unique. We respect the lifestyle, values and wishes of the patients, their families and caregivers.

    Many hospice patients wish to remain at home. However, Liberty Hospice can also provide services in an Assisted Living, Personal Care or Skilled Nursing community. We can help you to determine the best setting for the patient and his/her family.
