Liberty Hospice is proud of our dedicated team of professionals who will manage and oversee your care or the care of your loved one. This team will become an extension of your family during this difficult time.

You’ll get to know each other on a personal basis. Bonds will be formed and you’ll never feel uncomfortable asking a question or requesting something to comfort you or your loved one. You’ll establish a trusting and easygoing rapport with your Home Hospice team.

It’s a team that has one goal in mind: to respect and respond to the unique differences in lifestyles, values, and wishes of our patients, as well as provide unlimited support to the families and caregiving friends of those facing a life-limiting illness.

Pet Therapy is proven to reduce stress and anxiety, help with pain tolerance, promote social interaction and bring a great big smile to a patient’s face!

Music and Memory (sm) helps patients suffering from cognitive and physical challenges by finding renewed interest in their lives through the gift of their favorite music.

    Our Hospice Medical Director will work with an individual’s personal physician to develop a plan for pain and symptom management that focuses on a patient’s unique needs.

    Our specially trained Registered Nurses provide skilled care for pain and symptom management. They will also provide education and emotional support.

    Our Hospice Aides are available to assist with personal care needs and light housekeeping.

    Our Social Worker is on hand to provide ongoing emotional support and guidance, as well as to assist patients and families with identifying and connecting with community resources.

    Patients and families can depend on our Chaplain, Karynjean Dickhoff, to offer spiritual support and, if desired, work in tandem with your clergy and congregation of choice.

    Karynjean has more than a decade of experience in pastoral care and counseling, and crisis care and management. Her distinguished career includes chaplaincy roles in senior living, health care, and campground ministry.

    In addition to serving in the U.S. Army, Karynjean has a broad educational background that includes a bachelor’s degree in justice and advanced degrees in religion, theology, and teaching.

    Karynjean uses her rich and varied experience to bring comfort to patients and their families, facilitate grief support groups, and serve as an officiant for Liberty Hospice’s We Honor Veterans program, in which veterans at end of life are celebrated and thanked for their service.

    Our Bereavement Specialists are available to provide counseling, memorial services and other ongoing supportive measures to family and friends.

    Volunteers are a vital part of serving our hospice patients. By providing companionship and just being there to listen and talk, specially trained volunteers help us carry out our tradition of compassionate service to families and patients.

    Our volunteers are able to provide respite care for families, offering emotional support and helpful assistance with small errands and more.
