The mission of Liberty at Home Health is to provide exceptional health care services to adults and seniors who require professional care or support for a designated period of time in their place of residence. We promote spiritual, physical, and psycho-social wholeness. We accept payment from Medicare, Medicaid, and many other health insurances.  Our services are provided by a multi-disciplinary team of professionals including:

Skilled Nursing Care

Liberty at Home registered nurses and licensed practical nurses provide care for adults over the age of 18 in their homes. Available services include: ostomy care, incontinence programs, cardiac rehabilitation, diabetes management, mental health, injections, post-surgical care and medication teaching. Our nurses are also certified in wound care.

Home Health Aides

Home Health Aides provides services such as assistance with bathing, using the bathroom, dressing or other personal care needs as directed by a skilled professional.

Physical Therapy

Physical therapy is provided as prescribed and directed by a physician. Physical therapy includes performing exercises to regain movement and strength in a specific area of the body, as well as training on how to use special equipment or how to execute daily activities such as moving in and out of a bathtub or wheelchair safely.

Speech and Occupational Therapy

Speech language therapy involves exercises for the purpose of regaining and strengthening speech skills. Occupational therapy consists of retraining a patient to take on daily tasks, such as dressing, bathing and showering.

Social Work Services

Medical Social Work Services are provided to help patients with the social and emotional concerns they may have in relation to their illness. Services may include counseling or education about community resources.
