The many transitions that accompany aging may occur suddenly, or slowly over time. Liberty at Home’s registered nurses will help you through the steps necessary for moving through the decision-making process about your care at home.

When you call Liberty at Home, you will speak to one of our nurses about your home situation and schedule a time for an initial home assessment. This assessment consists of a series of questions and observations made by one of our nurses who will listen carefully to the older adult and family members involved in the care and decisions for in-home services.

The assessment visit will take about one hour. During this time, you will be encouraged to ask questions about scheduling, staff credentials, fees and the date that your care will begin. Liberty at Home does not charge a fee for this initial in-home assessment.
