When you call Liberty at Home to inquire about our Home Care Services, we will schedule a free home assessment visit with you. This visit will consist of a series of questions and observations made by one of our nurses who will listen carefully to you and your family members regarding your in-home service options.

During this assessment, we will gather information to assist you, including:

• Basic demographic information: name, date of birth, phone number, emergency contacts, etc.

• Your current medical condition

• Personal challenges or struggles you may have at present: personal tasks, meal preparation, laundry, medication, housekeeping, home maintenance, transportation the need for specialized services, etc.

• A personalized care plan will be created with you and your loved ones.
