The many transitions that accompany aging may occur suddenly or slowly over time. Liberty at Home’s registered nurses will guide you through the steps that are necessary for moving through the decision-making process regarding your personal at-home care.
Skilled Services (Liberty at Home Health)
After Liberty at Home receives an order for our services from your physician, we will call you to schedule your first visit with us. During this visit, a registered nurse will review your medical history and your physician’s prescribed services with you, as well as a number of informational forms, insurance information and the agreement and consent form that allows Liberty at Home to begin your services. In addition, you will be given a Patient Handbook describing our services, emergency procedures and other important information that you can easily access.

Home Care, Personal Care and Support Services (Liberty at Home Care)
When you call Liberty at Home to inquire about our Home Care Services, we will schedule a free home assessment visit with you. This visit will consist of a series of questions and observations made by one of our nurses who will listen carefully to you and your family members regarding your in-home service options.
During this assessment, we will gather information to assist you, including:
- Basic demographic information: name, date of birth, phone number, emergency contacts, etc.
- Your current medical condition
- Personal challenges or struggles you may have at present: personal tasks, meal preparation, laundry, medication, housekeeping, home maintenance, transportationm the need for specialized services, etc.
- A personalized care plan will be created with you and your loved ones.
