Liberty at Home Announces Medicare Certification!
Liberty at Home is proud to announce their new status as a fully Licensed and Certified Home Health Care Agency. Since its inception in 2004, Liberty at Home has provided non-skilled home care to individuals living throughout Southeastern Pennsylvania.
In addition to providing support and private duty care, Liberty at Home is now certified to serve individuals who qualify for home health services covered by Medicare.
To qualify for Medicare home health care services you are required to be:
- Under a physician’s care and plan of treatment
- Confined to home and unable to leave without considerable difficulty for more than short periods of time
- In need of a skilled professional to provide care: a Registered Nurse, Physical, Occupational or Speech Therapist.
- A person with a Medicare Card
- Using a Medicare Certified home care agency
Liberty at Home, Division of Home Health Services, offers skilled services which are covered by Medicare. These services are provided to adults and seniors needing care for a short period of time, are directed by a physician and delivered by an interdisciplinary team and include:
- Skilled nursing services that may include wound care, ostomy care, incontinence program, cardiac rehabilitation, diabetes management and mental health.
- Home heath aide services
- Physical, Speech and Occupational Therapy services
- Medical social worker services
- Durable medical equipment services
- Outpatient therapy when being seen by a home health nurse
- Respiratory care services
- Certain medical supplies needed for your care
Private duty and non-skilled support services are also available through Liberty at Home, Division of Home Care, and include:
- Certified Home Assistants for personal care needs and assistance with daily living
- Transportation Services for medical appointments, shopping and social engagements
- Lifestyle Care counseling for professional evaluation of support needs and resources available during transitions, such as retirement living options.
- Personal Household Management for bill paying, record-keeping, coordination of insurance benefits and completion of reimbursement forms.
- Nutritional Counseling for meal planning for specific dietetic needs, weight management and grocery shopping
- Home Maintenance and Repair Services for basic, skilled or seasonal work such as lawn care or snow removal.
